스마트 재활 솔루션 스타트업 네오펙트 (대표: 반호영 www.neofect.com )는 발달 장애 아이들의 재활훈련을 위한 ‘라파엘 스마트 키즈’를 출시했습니다. 라파엘 스마트 키즈는 2016년 10월  28, 29일에 열리는 대한재활의학회 추계학술대회 및 대한작업치료학회 추계학술대회에서 선보일 예정입니다.

라파엘 스마트 키즈는 ‘라파엘 스마트 글러브’의 소아용 버전으로 뇌성마비, 소아마비, 지체 장애 등 손 재활이 필요한 발달 장애가 있는 4세부터 13세 아이들을 위해 개발됐습니다. 이 제품은 아이들의 발달 단계에서 필요한 움직임 특히 능동적인 손 기능 향상을 위한 재활 훈련을 게임으로 할 수 있는 의료기기입니다.

아이들에게 놀이활동, 학습활동, 일상 생활 활동이 매우 중요함에도 불구하고 발달상의 어려움을 가진 아동들은 이러한 활동에 제한이 생깁니다. 라파엘 스마트 키즈의 재활 훈련 게임은 놀이, 학습, 일상 활동과 관련된 소재가 기반이 되며 지속적으로 반복 훈련을 하도록 개발됐다. 반복 훈련은 아동의 발달 단계에서 필요한 움직임의 향상 및 뇌 가소성을 증진시키는데 중대한 역할을 하게 됩니다. 더불어 라파엘 스마트 키즈는 아동들이 흥미를 가질 수 있는 다양한 재활 게임으로 구성돼 장시간의 훈련이 가능합니다.

또한 이번 제품은 자동 난이도 알고리즘(Learning schedule algorithm)을 기반으로 각 어린이 환자의 움직임 범위에 따라 게임 난이도가 조절됩니다. 데이터가 저장되고 그래프로 표현되는 훈련 결과를 제공하기 때문에 효율적인 재활 관리가 가능합니다.

연세 세브란스 병원 재활의학과 나동욱 교수님은 

“소아 재활의 대상이 되는 아동들 중에는 손 기능이 떨어지는 아이들이 많으며 불편한 손을 최대한 많이 사용할 수 있도록 하는 치료가 매우 중요합니다. 컴퓨터 게임에 익숙하고 흥미를 가지고 있는 아이들의 특징 때문에 게임으로 재활 훈련을 할 수 있는 라파엘 스마트 키즈는 아이들에게 동기부여를 할 수 있을 것으로 보입니다.”라고 말씀해주셨습니다.

뇌에 대한 연구는 아주 오래전부터 진행 되어 왔습니다.

초기에는 개의 뇌를 가지고 연구를 진행 하였습니다. 대표적으로 Frisch 와 Hitzig 라는 분들이 있습니다.

이분들은 개의 뇌를 가지고 대뇌 좌우 반구에서 오른쪽 대뇌반구는 왼쪽 사지를 왼쪽 대뇌반구는 오른쪽 사지를 움직인다는 것을 처음으로 증명 하였습니다.

초기에는 강아지와 고양이를 대상으로 주로 연구를 진행 하였고 이후에는 사람과 비슷한 영장류를 대상으로 한 연구를 진행 하였습니다.

대표적인 연구자로는 Sherrington 이 있습니다. 원숭이를 대상으로 연구 하였으며 원숭이의 대뇌 피질의 운동영역에 전기적 자극을 통해 각 움직임의 영역을 증명 하였습니다.

그리고 최근에는 Sherrington의 제자이기도 한 Penfield라는 연구자가 원숭이를 대상으로 하였던 움직임에 대한 뇌영역에 대한 연구를 사람을 대상으로 진행하였습니다.

이러한 연구들을 토대로 사람의 대뇌겉질의 운동영역이 대부분 증명 되었으나 새로운 의문이 생기기 시작했습니다. 운동영역의 움직임이 개별 근육을 조절을 위한 것인지 아니면 특정 동작을 위한 다수의 근육을 조절하는 것인지에 대한 의문이 생기게 되었습니다.

원숭이에게 쥐기 동작 중 섬세한 쥐기와 강한 쥐기 두 가지 동작을 취하게 하고 근육의 활성도와 대뇌겉질의 움직영역의 활성도를 측정 하였습니다.

연구 결과 강한 쥐기와 섬세한 쥐기 동작에서 근육의 활성도에서는 차이를 보이지 않았으나 대뇌겉질의 움직임영역의 활성도에서는 섬세한 쥐기에서 대뇌겉질의 활성도가 강한 쥐기 보다 많은 활성도를 보였습니다. 결과적으로 뇌의 대뇌겉질의 움직임영역의 활성화 또는 변화를 위해서는 단순한 동작 보다는 섬세한 동작을 하여야 뇌의 변화를 보다 많이 이끌어 낼 수 있다고 할 수 있습니다.

근력의 강화만을 목적으로 한다면 단순한 동작도 효과 가 있지만 뇌의 변화를 이끌어 내기 위해서는 섬세한 동작을 하여야 더 효과적이라고 할 수있습니다.

미국가전협회(Consumer Electronics Association)는 매년 1월 초에 열리는 세계 최대 가전전시회 CES의 출품작들을 대상으로 총 28개 부문에서 기술, 디자인, 소비자 가치 등을 평가해 ‘CES 혁신상’을 수여합니다. 라파엘 스마트 글러브는 ‘피트니스, 스포츠 및 바이오 테크’ 분야에서 선발 됐습니다.

라파엘 스마트 글러브는 뇌졸중 등 중추신경계질환 환자들이 다양한 재활 훈련 게임을 통해 손가락과 손목, 아래팔 기능의 재활 훈련을 할 수 있도록 개발된 치료용 의료기기입니다. 약 40여 개의 재활 훈련 게임은 환자들의 시각과 청각 두 가지 감각을 동시에 자극해 뇌 가소성 증진과 뇌 운동 부위의 재 학습을 도와줍니다. 또한, 훈련 중 측정되고 분석된 데이터는 인공지능을 통해 환자 상태에 맞는 다양한 과제훈련게임을 제공함으로써, 동기부여 되도록 디자인 됐습니다.

라파엘 스마트 글러브 ‘게임으로 재활훈련’
네오펙트는 지난 2014년 8월, 12월에 KFDA, 미국 FDA에 라파엘 스마트 글러브를 등록한 데 이어 2015년 10월, 의료기기 국제 품질관리 기준 인증인 ISO 13485를 획득해 제품 생산, 공정, 고객 관리 등에 관한 국제 표준 시스템을 구축 한 바 있습니다.

또한 지난 3월, 국립재활원은 해외 논문 JNER (Journal of Neuro Engineering and Rehabilitation. 2016)을 통해 전통 작업 치료와 손, 손목 및 아래팔 재활 훈련용 ‘라파엘 스마트 글러브’를 병행한 가상현실 기반 재활치료가 전통적인 재활치료만 한 것보다 더욱 효과적이며 이를 통해 손, 손목, 아래팔 등의 기능 향상과 건강한 삶의 질을 높이는데 기여한다는 것을 입증 했습니다.

네오펙트(대표 반호영)의 재활 훈련 솔루션 ‘라파엘 스마트 글러브’는 뇌졸중 등 중추신경계질환 환자들이 다양한 재활훈련 게임을 통해 손가락과 손목, 아래팔 기능의 재활훈련을 할 수 있도록 개발된 의료기기이다.

약 40여 개의 재활 훈련 게임은 환자들로 하여금 비디오 게임을 한다는 느낌으로 재미있게 시각과 청각 두 가지 감각을 동시 자극해 뇌 가소성 증진과 뇌 운동 부위의 재학습을 돕는다.

또한 훈련 중 측정되고 분석된 데이터는 인공지능(AI)을 통해 환자 상태에 맞는 다양한 과제훈련게임을 제공함으로써, 동기부여가 되도록 디자인됐다.

스마트 글러브의 명칭 라파엘은 ‘인간들의 고통을 치유하는 상냥한 마음의 천사 이름’을 가져왔다. 제품이름에 ‘치료’라는 단어를 넣고 싶었고 다양한 언어로 찾는 와중에 히브리어에서 ‘라파엘’이라는 찾게 됐다는 설명이다.

반호영 대표는 “역사에 대한 관심이 많고 고전을 즐겨 읽었기 때문에 히브리어와 라틴어 등에서 영감을 받아 이름을 짓게 됐다”며 “라파엘이란 이름은 뇌졸중 환자, 발달 장애 아동 등 재활훈련이 필요한 환자들을 재활훈련으로 치유하겠다는 의미를 담고 있다. 영어로 원래 스펠은 Raphael이지만 조금은 특별하게 Rapael이라고 쓰고 있다”고 설명했다.

회사명 네오펙트도 그리스어에서 유래한 접두어 ‘새로움’을 뜻하는 NEO와 효과를 뜻하는 Effect가 합성돼 만들어 ‘새로운 효과를 탄생시킨다’는 의미를 담아 탄생한 것도 눈길을 끈다.

스마트 글러브 통해 재활훈련을 하고 있는 모습

라파엘 스마트 글러브를 낀 뇌졸중 환자들이 화면으로 보이는 게임을 하면서 손을 펴고 쥐고 구부리는 연습을 한다. 컵을 잡거나 페인트를 칠하고, 오렌지를 짜거나 공을 잡고 던지는 게임을 되풀이하면서 일상생활에 필요한 움직임을 익히는 훈련을 한다.

건강한 사람에겐 쉬운 동작도 뇌졸중 환자에겐 어려운 일이라 끊임없이 반복하며 훈련을 할 수 있게 만들고 라파엘 스마트 글러브는 센서를 통해 게임을 하는 환자의 움직임 민감하게 측정하고 분석해서 통합적인 데이터를 제공한다.

한편 네오펙트는 지난 10월 발달장애 아이들의 재활훈련을 위한 ‘라파엘 스마트 키즈’를 출시했다.

이 제품은 라파엘 스마트 글러브의 소아용 버전으로 뇌성마비·소아마비·지체 장애 등 손 재활이 필요한 발달 장애가 있는 4세부터 13세 아이들을 위해 개발됐으며 아이들의 발달단계에서 필요한 움직임 특히 능동적인 손 기능 향상을 위한 재활 훈련을 게임으로 할 수 있도록 디자인됐다.

거울신경세포의 발견

하품이 전염되어 본 적이 있습니까? 모두들 한번쯤은 입이 찢어져라 하품하는 모습을 보고 자신도 모르게 하품을 따라서 해본적이 있을 것입니다.  또 마주 앉아 이야기를 나누고 있는 사람의 얼굴의 표정 또는 얼굴의 미세한 근육 변화를 내 얼굴에서 똑같이 일어나는 것을 느껴 본 경험이 있을 것입니다.

마치 거울에 비친 것처럼 말입니다. 이 모든 현상은 우리의 의지와는 상관없이 불수의적(자동적)으로 일어납니다. 불수의적이든 수의적이든 간에 근육이 수축하고 동작이 일어났다는 것은 우리 중추신경계에서는 그와 관련된 운동 프로그램이 실행된것입니다.

어째서 이러한 현상, 다른 사람의 움직임이나 동작을 보고 나도 모르게 따라서 하는 현상이 일어날까요?

이를 신경생리학적으로 설명해 주는 것이 바로 ‘거울신경세포(mirror neuron)’입니다.  거울신경세포란 다른 사람이 어떤 동작을 실행하는 것을 관찰할 때나 자신이 그 동작을 실행할 때에도 활성화되는 신경세포를 말합니다. 마치 거울에 비친 것처럼 활성화된다고 하여 붙여진 이름입니다.

신경과학 역사에서 가장 획기적인 발견으로 평가받는 이 거울신경세포 연구는 이탈리아의 아인슈타인이라고 불리는 파르마 대학의 신경과학자 Giacomo Rizzolatti는 거울신경세포에 대해서 1995년 처음으로 증명(발견) 하였습니다.

모든 위대한 발견이 그렇듯이, 거울신경세포 존재도 아주 우연한 사건으로 발견되었습니다. 파르마 대학의 Giacomo Rizzolatti의 실험실에서 원숭이를 대상으로 한 뇌의 운동영역에 대한 연구를 진행하였습니다. 원숭이의 대뇌의 Ventral premotor cortex에 전극을 삽입한 후 음식을 손으로 집어서 먹는 과제를 주었습니다. 과제를 수행하는 동안 원숭이 대뇌에 삽입한 Ventral premotor cortex가 활성화 되면 벨소리와 점자로 기록이 되었습니다.

연구를 하는 중간의 쉬는 시간에 연구팀의 팀원 한명이 장난삼아 실험중인 원숭이의 음식을 하나 뺏어 먹었습니다. 그런데 벨소리와 점자 기록이 표시 되었습니다. 연구원은 깜짝 놀라서 Giacomo Rizzolatti에게 이 사실을 보고 하였습니다.

Giacomo Rizzolatti는 이러한 반응에 대해 자세히 연구하기 시작 했습니다. 위의 그림처럼 세 가지 상황을 주고 반응을 대뇌 Ventral premotor cortex의 반응을 비교 하였습니다.

비교 결과 세 가지 상황에서 모두 비슷한 뇌의 활성도가 나타났습니다. 이에 근거하여 Giacomo Rizzolatti는 대뇌 Ventral premotor cortex( F5) 운동영역에는 동작을 보는 것만으로도 반응하고 활성화 하는 거울신경세포가 존재한다고 발표 하였습니다.

A recent study completed by the University of California, Irvine, reported that individuals who had experienced a stroke, saw as much improvement in their mobility through telerehabilitation sessions, those provided with coaching through video calls over the computer and other specialized tracking tools, as those traveling to complete rehab in-person.

The study, presented at the 2018 European Stroke Conference in Sweden, was a multicenter trial involving over 120 stroke survivors as participants. Study participants were an average of 61 years old and 4.5 months post stroke. They completed 6 weeks of intensive hand and arm therapy, half in a clinic setting and half in a supervised program at home via a telerehab video system.

Both groups improved in their motor function scores, the clinic group saw an 8.4 point increase and the telerehab group saw a 7.9 point increase on the Fugl Meyer, a standardized assessment of hand and arm function. This wasn’t a statistically significant difference but an increase in function, nonetheless, and very close to the same increase the clinic group experienced.

With an aging population, it is thought that the need for this type of service will only increase over time. Future studies plan to look at longer term treatment and including gait and language.
